To continue receiving quarterly reimbursements for your Medicare Part B Premium, Zenith American Solutions (CTA Health and Welfare Benefits Trust Administrator) sent letters to Medicare eligible retirees in January with instructions regarding their new Medicare reimbursement process. The letter stated that eligible retirees and spouses, if applicable, were required to submit a one-time affidavit (see attached) and a copy of their red, white, and blue Medicare card to the Trust by March 1, 2024.
REACTA recently received a list of retirees and/or spouses who Zenith has recorded as NOT having received the one-time Affidavit and copy of your red, white, and blue Medicare card by the March 1, 2024, deadline.
REACTA believes that Zenith has made errors on this list. However, if you did not submit this information to Zenith, it is IMPORTANT that you do so without delay. The one-time affidavit and copy of your red, white, and blue Medicare card should be mailed to:

Zenith will be sending out reminder letters only to this group of retirees which will include a new return due date. If you receive such a letter, it is important that you follow their instructions, even if you believe that you have already complied the first time. As the saying goes, “It’s better to be safe than sorry”. Or contact Zenith directly at 888/243-2325 to verify whether they have received your documents.
The one-time affidavit due date is extended to April 19.
REACTA will continue to monitor the situation and work with Zenith as they continue to update their records.
Update March 28, 2024
If you recently received a second notification from Zenith that they have not received your signed Medicare Part B Reimbursement Affidavit and Medicare Card, it is because, according to their records, you did not meet the March 1st deadline to return the documents. We know that, in a lot of cases this is not true, because we have communicated with many of you. Some have sent it in three times! Others sent both theirs and their spouse's documents in the SAME envelope, yet only one or none were recorded.
If you received the NEW notice and you have already sent in your documents, call Zenith to verify that they NOW have recorded that it was received. If they say they still do NOT have it, I suggest that you send it again.
After the new deadline of April 19th, REACTA should receive a new and hopefully more accurate list. When we receive that new “delinquent” list, we will reach out to those specific REACTA members.
Zenith clearly was unprepared to deal with the compilation of hundreds of affidavits. Retirees far outnumber active employees, who are not Medicare recipients. However, if there is a light at the end of the tunnel, once this process is finally completed, we will no longer have to do the annual ritual of submitting our Social Security Benefits Statement.
